Job summary

Boost Home Healthcare seeks a dedicated Case Manager (RN) to deliver skilled nursing care in patients’ homes under physician orders and the plan of care. You will operate under the Director of Patient Care Services, evaluating nursing needs and coordinating revisions to the care plan.

Qualifications

• Graduate of an accredited school of professional nursing.
• Currently licensed as a registered nurse in the state of the job posting.
• Minimum of one (1) year of nursing experience in a home health setting.
• Must have current BLS Certification.
• Access to reliable transportation.

Responsibilities

• Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Meet Daily Productivity Standards.
• Initiate the plan of care and necessary revisions.
• Provide those services requiring substantial specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.
