Jobot is seeking an RN Case Manager in Roscoe, NY. This on-site, permanent role handles coordination between physicians, the skilled nursing team, and families to ensure seamless continuity of care.

• The RN Case Manager will serve as a liaison between physicians, skilled nursing team and family members to ensure continuity of care.
