Jobot is seeking an RN Clinical Coordinator - Specialized Care Planning to join our team in Liberty, NY. You will monitor quality patient care in a Skilled Nursing Facility and coordinate ongoing services, serving as a liaison between physicians, the nursing home team, and families.

The ideal candidate holds an active NYRN license and SNF/LTC or Home Health experience, with knowledge of CMS regulations and excellent interpersonal communication skills to ensure high-quality outcomes.

Seeking a Registered Nurse to work in a Skilled Nurse Facility monitoring quality patient care and coordinating ongoing services. Serve as a liaison between physicians, nursing home team, and family members to promote quality patient care.
