Job summary

Community Care, Inc. is hiring a RN Care Manager to join our Family Care Program team in Wisconsin. This full-time role involves collaborative community case management to help older adults and individuals with disabilities live independently in the community. A sign-on bonus of $8,000 is offered.

The RN Care Manager will complete assessments, develop care plans, and coordinate services with families and supports to ensure quality outcomes. Monday–Friday, 8:00 AM–4:30 PM in Ozaukee County.

Qualifications

• Associates degree in Nursing required.
• Bachelor's degree preferred.

• Completes discipline-specific initial and ongoing assessments; conducts home visits and home assessments, consulting with families and supports as needed.
• Develops and maintains care plans, including long-term care/outcomes; follows up on services to ensure quality, coordination and effectiveness.

RN Care Manager Job Responsibilities:

• Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.
• Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.

RN Care Manager Job Requirements:

• Education: Associates degree in Nursing Required, Bachelors Degree preferred.
• Licensure: Wisconsin Licensed Registered Nurse
• Licensure: Eligibility for long-term care functional screener certification is required
• Licensure: WI Drivers License
• Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required
  Preferred 2 years of work in community nursing

RN Care Manager Job Schedule:

• Monday to Friday
• 8:00AM - 4:30PM
