Job summary

Community Care is seeking a Case Manager/Care Manager to join our Partnership Program team in Racine County. This full-time position offers a Monday–Friday schedule with a $5,000 sign-on bonus and a mission-driven focus on helping elderly and disabled adults live independently in the community.

Responsibilities include case load management, assessments, care planning, care coordination, and documentation in line with standards.

Qualifications

• Bachelor's degree in a Human Services field required.
• 3 years’ experience with seniors or adults with disabilities may substitute for a non-human services degree.
• Eligibility for long-term care functional screener certification required.
• Regular local travel with valid driver's license and insurance.

This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position based in Racine county.

Care Manager/Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Care Manager/Case Manager Job Requirements:

• Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
