Job summary

Community Care, Inc. in Wisconsin is seeking a Case Manager to join our mission-driven team in Waukesha County. This full-time position offers a hybrid weekday schedule and an $8,000 sign-on bonus.

You will manage caseloads with an RN Case Manager, conduct comprehensive assessments, and develop care plans while coordinating services and maintaining documentation to high standards. Local travel is required.

Qualifications

• Bachelor's Degree in a Human Services or closely related area required.
• Three years’ experience with seniors or adults with disabilities may substitute for a non-human services degree.
• One year of experience with older adults or persons with disabilities in a social service capacity preferred.
• Eligibility for long-term care functional screener certification is required.
• Local travel requires use of a personal vehicle, valid driver's license, and proof of insurance.

Responsibilities

This full-time opportunity offers a hybrid weekday schedule and a $8,000 SIGN-ON BONUS. Position located in Waukesha County.

Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Case Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager Job Requirements:

Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.

Experience: One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred.

Certification: Eligibility for long-term care functional screener certification is required

Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
