Job summary

Community Care, Inc. is hiring a Case Manager to join our mission-driven team serving Washington and Dodge Counties in Wisconsin.

This full-time role focuses on community-based case management, care coordination, and collaborating with an RN Case Manager to support seniors and adults with disabilities. The position offers a Monday–Friday schedule (8:00 AM–4:30 PM) with a $5,000 sign-on bonus and a comprehensive benefits package.

Qualifications

• Bachelor's degree in a Human Services or closely related area required; a minimum of three (3) years' experience with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human services degree.
• Eligibility for long-term care functional screener certification is required

Responsibilities

• Manages an assigned caseload of program members in collaboration with an RN Case Manager.
• Completes comprehensive member assessments, member centered care plans, and functional screens.
• Provides ongoing care coordination and quality assurance monitoring of services.
• Maintains required documentation in adherence to established standards.

Case Manager Job Responsibilities

• Manages an assigned caseload of program members in collaboration with an RN Case Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager Job Requirements

• Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years' experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
