Actalent, Inc. in Milwaukee, WI is seeking an experienced RN Care Manager to conduct comprehensive assessments and develop member-centered care plans for adults, including those with disabilities.

This permanent role supports home and facility visits, coordinating ongoing care while ensuring quality and respect in all member interactions. The position offers a hybrid schedule, a weekday hours, mileage reimbursement, generous benefits, and a sign-on bonus.

Qualificações

• Wisconsin RN license required.
• 1+ year of care/case management experience required.
• 2+ years of experience with geriatric population or individuals with mental/physical disabilities.

Responsibilities

• Conduct clinical assessments and develop member-centered care plans.
• Coordinate ongoing care, ensuring quality assurance and monitoring services for program members, including older adults and individuals with intellectual or physical disabilities.
• Achieve member outcomes and goals by adhering to standards of care and best practices.
• Interact with members and their families with the utmost respect, compassion, and integrity.
• Identify and participate in continuous improvement efforts to enhance the quality of member care and services, fostering a member-centered culture daily.
• Engage in efforts to improve work experiences and actively seek training and development opportunities.
• Support operational goals by adhering to standards and complying with contractual, regulatory, and CCI policy requirements.
• Complete initial and ongoing comprehensive assessments specific to the discipline.
• Conduct home visits and assessments as needed, consulting with families and other supports for additional information.
• Utilize critical thinking to optimize members' health and quality of life, helping them reach their goals.
• Develop and maintain care plans.
• Educate members and their families.

Qualifications

• Wisconsin Registered Nurse License
• 1+ year of Care/Case management experience
• 2+ years of experience working with geriatric population or intellectuals with mental and/or physical disabilities.

Additional Information

This role offers a flexible hybrid schedule. It is Monday-Friday, 8am-4:30pm. No holiday or weekend requirement. Job Type & Location

This is a Permanent position based out of Milwaukee, WI.

Pay and Benefits

The pay range for this position is $35.00 - $38.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
