Job summary

Community Care, Inc. is seeking a RN Case Manager to join our Family Care Program in West Bend, serving Washington and Dodge Counties. This full-time role includes home visits, care plan development, and coordination of services to help seniors and adults with disabilities live independently in the community.

The ideal candidate holds an Associate's degree in Nursing with an active Wisconsin RN license and at least 2 years of relevant experience. A $8,000 sign-on bonus is offered.

Qualifications

• Associates degree in Nursing is required; Bachelor's degree preferred.
• Wisconsin RN license is required.
• Eligibility for long-term care functional screener certification is required.
• WI Driver's License is required.
• Minimum 2 years of nursing experience with older adults or persons with disabilities in community or clinical settings.

Responsibilities

• Performs initial and ongoing discipline-specific assessments, conducts home visits, and communicates with families and support networks.
• Creates and maintains comprehensive care plans, tracks progress toward outcomes, and follows up on services to ensure quality and coordination.
