The role involves clinical oversight, care planning, and working with patients and families, under the supervision of the RN Clinical Manager. Ideal candidates have Home Health experience, CPR, and a valid Wisconsin license.

Qualifications

• Active Wisconsin Registered Nurse License.
• Two years nursing experience, with 1 year in Home Health preferred.
• CPR Certification required.
• Valid Wisconsin Driver's License and car insurance.

Responsibilities

• Provide guidance and oversight in clinical and regulatory matters.
• Coordinate services and counsel the patient and family in their home.
• Review care plans and conduct incident investigations.

What does a Transitions at Home RN Case Manager do?

Transition at Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and Who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full - Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• Weekends or Holidays
• Limited on-call rotation
