Transitions At Home for Western Wisconsin is actively hiring a Full-Time RN Case Manager in the Sun Prairie area. Ideal candidates hold an active Wisconsin RN license, have home health nursing experience, CPR certification, and a valid Wisconsin driver's license with car insurance.

Qualifications

• Active Wisconsin Registered Nurse License
• Two (2) years’ nursing experience, 1 year in Home Health (preferred)
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance

Responsibilities

• Provide skilled nursing care in home including assessments and treatments.
• Coordinate services and medications per physician orders.
• Counsel patient and family in their residence per plan of care.
• Report to RN Clinical Manager and Administrator as required.

We're hiring! Transitions At Home, is currently seeking to hire a Full-Time RN Case Manager in the Sun Prairie area. Our growing home health agency is seeking a compassionate and experienced full-time RN Case Manager to join our team. In this role, you’ll coordinate and oversee patient care in the home setting, ensuring personalized, high-quality outcomes for every client.

What does a Transitions at Home RN Case Manager do?
Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.

Hours: Full-Time

Paid Drive Time - Company Vehicle

• Monday through Friday 8am-430pm
• Limited on-call rotation
