Beacon Hospice, part of the Pennant Group family, is seeking an RN Case Manager to join our hospice team in Jackson, TN. This role delivers comprehensive nursing care, coordinates plans of care, and supports patients and families on the journey toward meaningful comfort and quality of life.

The RN Case Manager will manage visits, documentation, and multidisciplinary collaboration with physicians, IDG members, and community resources. Travel is required to serve multiple residences daily.

Qualifikationen

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, valid driver’s license and reliable vehicle.
• Must be able to travel to multiple patient residences daily.

• Make assigned patient care visits per physician orders and document accurately (visit reports, plan of care, progress notes, etc.).
• Attend patient meetings and provide input on care plans and progress toward goals.
• Adhere to agency policies and procedures; seek guidance as needed.
• Establish and maintain an efficient visit route for patients.
• Provide services within the scope of nursing practice as defined by state law and coordinate care with patients, families, and the IDG.

POSITION OVERVIEW:

The RN Case Manager is responsible for the full delivery of the plan of care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity, and improve the quality of care delivered. A few position highlights include:

• Make assigned patient care visits in accordance with physician orders. Complete supporting documentation accurately and timely, including visit report, OASIS assessment, plan of care and revisions, medication profile, progress notes, recertification, discharge/transfer summary, etc.
• Attend meetings for all patients assigned. Participate and offer input regarding each patient’s care and progress toward individual patient goals.
• Understand and adhere to all agency policies, procedures, and processes (administrative, personnel, clinical, etc.). Seek guidance and clarification when uncertainty/confusion arises.
• Establish and maintain an effective and efficient visit route.
• Provide services within the scope of practice as defined by the state laws governing the practice of nursing, initiates and coordinates the plan of care, collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.

THE MUST HAVES:

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
