Job summary

Beacon Home Health in Jackson, TN is seeking an experienced Registered Nurse to join our Home Health team. The RN provides and directs nursing care to patients in their homes per physician orders and agency policies.

Responsibilities include creating and coordinating plans of care, conducting initial evaluations, maintaining documentation, and delivering high-quality care from admission to discharge with travel to patient residences within the assigned area.

Qualifications

• At least 1 year of clinical RN experience.
• Current RN license in Tennessee.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Create, implement, execute, and coordinate the plan of care to promote wellness and prevent rehospitalization in patients' homes.
• Perform initial/comprehensive nursing evaluations and determine the primary focus of care within state guidelines.
• Maintain accurate documentation and clinical notes from admit through discharge.
• Establish an efficient visit route and provide high-quality clinical services within scope of practice.
