Beacon Hospice in Jackson, TN is seeking a dedicated RN Case Manager to oversee care plans for hospice patients while on the road. You will perform visits, document care, and collaborate with physicians, families and the IDG to improve quality of life.

The role requires an RN license in TN, CPR certification, and reliable vehicle; you will coordinate with an internal Pennant team to deliver compassionate, compliant home-based care across the assigned geographic area.

Qualifikationen

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification and valid driver's license with vehicle access.
• Must be able to reliably commute to multiple patient residences daily.

Aufgaben

• Make assigned patient care visits in accordance with physician orders.
• Complete supporting documentation accurately and timely, including visit reports and care plans.
• Attend meetings for all patients assigned.
• Participate and input regarding each patient’s care and progress toward goals.
• Adhere to all agency policies, procedures, and processes.
• Establish and maintain an efficient visit route.
• Provide services within the scope of practice and coordinate the plan of care with the IDG.
