The RN Case Manager is responsible for managing and delivering the full plan of care for each home health patient assigned, while continuously evaluating personal and professional performance and making necessary changes to increase productivity, and improve the quality of care delivered.
A few position highlights include: Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes. To maintain continuity of patient care, plan and exchange information with physician, agency personnel, patient, family, and community resources. Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer. Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care. Establish and maintain an effective and efficient visit route. Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.
