Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate, patient-centered care on the road. You will conduct initial assessments, refine care plans, and coordinate across the care team to ensure comfort in the final days and weeks of life.

Qualifications

• Licensed RN in the state of employment.
• Experience with EMR software.
• Strong planning, organizing and communication skills.

Responsibilities

• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.

Responsibilities:

• Conduct initial client assessments and continuously refine care plans.

• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance
