Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate, patient‑centered care across the final days and weeks of life. You will assess needs, refine care plans, and coordinate with families and a multidisciplinary team while working closely with the company’s owners.

Qualifications

• Licensed as a Registered Nurse (RN) in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
