Job summary

Moments Hospice is seeking an experienced RN Case Manager to deliver compassionate, patient-centered care on the road. You will assess clients, plan and refine care, and coordinate with families and the healthcare team to ensure comfort in final days.

The role includes hands-on nursing, education for patients and families, interdisciplinary collaboration, and the benefit of a company car with fuel and insurance, plus comprehensive health coverage and retirement options.

Qualifications

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance

• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
