Job summary

Moments Hospice is seeking an RN Case Manager to provide compassionate in-home care, conduct assessments, and coordinate treatment with families and the care team.

This on-the-road role comes with a company car including gas and insurance, comprehensive health, dental, and vision coverage, generous PTO, and a 401(k) plan with company matching. Salary ranges from $75,000 to $90,000 per year; on-call support is available to help manage workload and ensure high-quality patient care.

Qualifications

• Licensed RN in the state you work in and proficient with EMR software.
• Able to plan and coordinate patient care with strong communication.
• Autonomous problem solving while maintaining high standards.
• Valid driver's license and reliable vehicle with insurance.

Responsibilities:

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Qualifications:

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance
