Job summary

Moments Hospice is seeking an RN Case Manager to facilitate comfortable end‑of‑life care and work closely with company owners in a supportive, entrepreneurial environment.

You will assess patients, coordinate care, and guide families through complex transitions, with on‑the‑road responsibilities and a comprehensive benefits package.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands‑on nursing care and patient education.

Responsibilities:

• Valid Driver's License and reliable vehicle with current insurance
