Job summary

Moments Hospice in Florida seeks an RN Case Manager to assess patients, design and refine care plans, and coordinate with the care team to ensure comfort during final days. You will work closely with the company''s owners and contribute to growth.

This role includes hands-on nursing, patient education, on-call support, a fleet car with fuel and insurance, and comprehensive health benefits. A valid RN license and driver’s license are required to perform duties across the caseload.

Qualifications

• Licensed as a Registered Nurse in the practicing state.
• Experience with case management and care coordination.
• Strong communication and planning skills.
• Valid driver''s license and reliable vehicle.
• Proficiency with EMR systems.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Responsibilities:

• Valid Driver's License and reliable vehicle with current insurance
