Job summary

Moments Hospice is seeking an experienced RN Case Manager to provide compassionate end-of-life care and coordinate patient plans. You will work closely with patients, families, and the care team to ensure comfort and clear communication while supporting the entrepreneurial spirit of the organization.

Qualifications

• Licensed registered nurse in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families and care team.

Responsibilities:
