Job summary

JourneyCare Hospice is looking for a Full-Time Hospice Registered Nurse (RN) Case Manager to provide compassionate, patient-centered care in Lake Forest, Waukegan, and Zion, IL. This role offers a consistent Monday–Friday schedule and only one weekend rotation every 12 weeks.

The position includes competitive compensation ranging from $80,000 to $97,000 annually based on experience, along with benefits such as 401(k) matching, comprehensive insurance, generous PTO, and support for hospice

Qualifications

• Must have a valid RN license in Illinois.
• Experience in hospice or palliative care preferred.
• Ability to work autonomously in the field.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.

• Deliver skilled nursing care focused on comfort and dignity.
