JourneyCare Hospice in Carol Stream, IL is seeking a compassionate Hospice Registered Nurse (RN) Case Manager for a full-time role. You will oversee a dedicated hospice caseload, perform patient assessments, admissions, and care planning to ensure comfort and dignity at end-of-life.

The position offers a Monday–Friday 8:30 AM–5:00 PM schedule with one weekend rotation every 12 weeks in the Carol Stream territory, plus competitive compensation and benefits.

Qualifications

• Active Illinois RN license and CPR/First Aid certification.
• Experience in hospice or home health care preferred.
• Strong clinical judgment and clear communication skills.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.

Key Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager
• Perform patient assessments, admissions, and individualized care planning
• Deliver skilled nursing care focused on comfort and dignity
• Coordinate care with physicians and interdisciplinary team members
• Conduct routine visits and adjust care plans as needed
• Provide end-of-life support, including family education and pronouncement
• Maintain timely, accurate documentation
• Supervise Home Health Aides in compliance with regulations

Qualifications

• Graduate of an accredited RN program
• Active Illinois RN license
• Current CPR and First Aid certification

• Valid driver’s license and proof of insurance
