Job summary

JourneyCare Hospice in Highland Park, IL is seeking a compassionate Hospice RN Case Manager to provide patient-centered end-of-life care. Schedule: Monday–Friday 8:30 AM–5:00 PM with one weekend rotation every 12 weeks; territory includes Highland Park and surrounding communities.

As the primary RN Case Manager, you will assess, admit, and plan individualized care, delivering skilled nursing care with comfort and dignity, coordinating with physicians and the interdisciplinary team, and

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Highland Park area.

Schedule & Territory

• Territory: Highland Park, IL 60037 and surrounding communities.
• Monday–Friday, 8:30 AM – 5:00 PM. No On-Call.
• One weekend rotation every 12 weeks.

Compensation & Benefits

• $80,000 – $97,000 annually (based on experience)
