JourneyCare in Northbrook, Illinois is seeking a Hospice Registered Nurse Case Manager to provide compassionate end-of-life care. This position offers manageable caseloads with strong interdisciplinary support and less frequent weekend rotations.

Schedule & Territory

• $80,000 – $97,000 annually (based on experience)

Key Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager
• Perform patient assessments, admissions, and individualized care planning
• Deliver skilled nursing care focused on comfort and dignity
• Coordinate care with physicians and interdisciplinary team members
• Conduct routine visits and adjust care plans as needed
• Provide end-of-life support, including family education and pronouncement
• Supervise Home Health Aides in compliance with regulations

Qualifications

• Strong clinical judgment and communication skills
