JourneyCare Hospice in Highland Park, IL is hiring a Hospice Registered Nurse Case Manager for a full-time role. You will manage a dedicated hospice caseload, perform assessments, admissions, and care planning to ensure comfort and dignity for patients at end of life.

Role emphasizes coordination with physicians and the interdisciplinary team, routine visits, and accurate documentation. A strong clinical background and IL RN license are required.

Qualifications

• Active Illinois RN license required.
• Current CPR and First Aid certification required.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills required.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.

Hospice Registered Nurse (RN) Case Manager – Full-Time | Highland Park, IL

At JourneyCare Hospice, we do things differently. We prioritize manageable caseloads, strong interdisciplinary support, and giving our nurses the time they need to truly care for patients and families.

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Highland Park area.

Schedule & Territory

• Territory: Highland Park, IL 60037 and surrounding communities.
• Monday–Friday, 8:30 AM – 5:00 PM. No On-Call.
• One weekend rotation every 12 weeks.

Compensation & Benefits

• $80,000 – $97,000 annually (based on experience)
