JourneyCare Hospice in Highland Park, IL is seeking a compassionate Hospice RN Case Manager to deliver patient-centered, end-of-life care. You will manage a dedicated caseload with interdisciplinary support and coordinate with physicians to create personalized care plans.

Work schedule is Monday–Friday, 8:30 AM–5:00 PM with no on-call. Salary ranges from $80,000 to $97,000 per year, plus a comprehensive benefits package and enrollment in company programs.

Qualifications

• Graduate of an accredited RN program.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Supervise Home Health Aides in compliance with regulations.

• $80,000 - $97,000 annually (based on experience)
