JourneyCare Hospice in Highland Park, IL seeks a compassionate Hospice RN Case Manager to provide patient-centered, end-of-life care. You will manage a dedicated caseload, assess and plan individualized care, and collaborate with physicians and the interdisciplinary team.

This full-time role offers a stable schedule (Mon–Fri, 8:30 AM–5:00 PM), no on-call, and competitive benefits including 401(k) with company match, medical/dental/vision, generous PTO and holidays, plus hospice certification

Qualifications

• Graduate of an accredited RN program.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong communication and clinical judgment.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Supervise Home Health Aides in compliance with regulations.

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Highland Park area.

Schedule & Territory

• Territory: Highland Park, IL 60037 and surrounding communities.
• Monday–Friday, 8:30 AM – 5:00 PM. No On-Call.
• $80,000 – $97,000 annually (based on experience)
• 401(k) with company match
