Job summary

The Ohio State University Wexner Medical Center is seeking a Clinical Care Manager to coordinate an assigned patient group through inpatient care and safe transitions to outpatient settings. Qualifications

• RN licensure and nursing registration required.
• Two years of recent clinical experience preferred.
• Familiarity with payer protocols and evidence-based practice.

Responsibilities

• Primary responsibility for coordination of post-discharge home care services.
• Coordinate patient care through inpatient to outpatient transitions.
• Collaborate with physicians, nurses and other healthcare staff to manage care.
• Monitor, document and communicate data to improve timeliness and appropriateness of care.
