Job summary

The Ohio State University Wexner Medical Center is seeking a Care Manager (nurse case manager) to guide patients from pre-admission through post-discharge coordination of care. The role focuses on planning, resource use, and ensuring timely decisions and high-quality care.

The candidate will collaborate with physicians, nurses and other health care workers to manage transitions, communicate with payers, and support appropriate levels of care using evidence-based criteria.

Qualifications

• RN licensure required and a bachelor’s degree in nursing or equivalent.
• Two years of registered nursing experience required.
• Five years of recent clinical experience in relevant population preferred.
• Knowledge of clinical practice guidelines, payer protocols and data bases desired.

Responsibilities

• Coordinate patient care from pre-admission through post-discharge.
• Collaborate with physicians, nurses and health care staff to manage transitions.
• Communicate plan of care and medical necessity to third-party payers and gain approvals.
• Identify and remove barriers to progression of the care plan.
• Utilize nursing process to enhance care quality, efficiency and throughput.
