Job summary

The Ohio State University Wexner Medical Center is hiring a Care Manager to lead clinical care coordination for OSU Health Plan members and affiliated groups. You will collaborate with care teams to ensure high-quality, affordable services across the care continuum.

You will manage acute and complex cases, support discharge planning, and advocate for patients and families while utilizing evidence-based guidelines. A RN license and nursing degree are required; CCM certification is preferred.

Qualifications

• Current State of Ohio RN license required.
• Bachelor's Degree in Nursing from an accredited program required; master's preferred.
• Certified Case Manager (CCM) or other CCM certification desired.
• 2 years of relevant experience required.

Responsibilities

• Manage a caseload of moderate to high-risk adults and pediatric patients with chronic conditions.
• Coordinate care across the health care continuum including hospitals, SNFs, home health, hospice.
• Discharge planning and coordination with care team and providers.
• Document treatment plans, utilization management, and outcomes.
• Provide guidance to junior team members and ensure high-quality, cost-effective care.
