Responsibilities

• Facilitates patients through the health care system from pre-admission through post-discharge coordination of care.
• Monitors and coordinates the plan of care to improve timeliness and throughput, ensuring care quality and efficient resource utilization.
• Identifies and removes barriers to progression of the plan of care for each patient.
• Collaborates with physicians, nurses and other health care workers to manage the transition to the next level of care.
• Acts as the central information source for third party payers, communicating the plan of care and gaining approval for post-discharge care.
• Assists physicians in ensuring the appropriate level of care through evidence-based criteria.
