The Ohio State University is seeking a Care Management RN - PRN for East Hospital. The Clinical Care Manager coordinates care across inpatient to outpatient transitions, working with physicians, nurses, and care teams to ensure timely and effective patient management. Required is a Bachelor’s in Nursing with RN licensure; two years of clinical experience preferred.

Masters is a plus. Applicable knowledge includes evidence-based practice, payer protocols, and technology proficiency.

Formación

• RN licensure required; two years of recent clinical experience preferred.
• Bachelor's degree in Nursing required; Master's degree preferred.
• Knowledge of evidence-based practice and disease-specific processes.
• Technology skills and familiarity with payer protocols are desirable.

Responsabilidades

• Coordinate care for an assigned group of patients through the health care system.
• Monitor patient care process and data to improve timeliness and appropriateness of care.
• Collaborate with physicians, nurses and other health care workers to plan and communicate care plans.
• Support efficient resource utilization and safe transitions from inpatient to outpatient settings.
