Job summary

Ohio State University Physicians is seeking a Clinical Nurse Care Coordinator to collaborate with clinic teams to deliver evidence-based, patient-centered care. The role emphasizes management of high-risk medically complex patients, care transitions, and communication with families and external agencies to ensure holistic care.

As an RN, you will stay current on standards, support program development, and help patients stay up to date with their care plans and goals.

Qualifications

• Associate Degree in Nursing from a CCNE/ACEN accredited program.
• Current Ohio RN license.
• CPR/BLS certification.
• Triage procedures knowledge in clinic/urgent care.
• Maintain quality, safety and infection control standards.
• Educate patients and families about disease and care.
• Clear written and verbal communication.
• Strong interpersonal, tactful and diplomatic skills.
• Analytical thinking to gather and interpret data and develop solutions.

Responsibilities
