Job summary

The Ohio State University in Columbus is seeking a Clinical Care Manager RN for the East Hospital Care Management Services to coordinate inpatient care and ensure a smooth transition to outpatient services.

You will collaborate with physicians, nurses, and other staff, monitor patient care processes, document plans, and use evidence-based practices to improve timeliness and outcomes while staying aligned with payer protocols.

Qualifications

• Bachelor’s degree in Nursing; master’s preferred.
• RN licensure; experience in a relevant clinical area preferred.
• Knowledge of evidence-based practice and disease-specific processes; technology support and payer protocols desired.

Responsibilities

• Coordinate care for an assigned group of patients across inpatient and transition to outpatient settings.
• Collaborate with physicians, nurses and other health care workers to assess, treat, and follow up.
• Monitor care processes, document data, and communicate plans of care to improve timeliness and appropriateness.
