Job summary

You will educate and empower patients and families, collaborate with the care team, and support safe transitions while focusing on reducing readmissions. Requirements include a Master’s in Nursing and several years of clinical and case management experience.

Qualifications

• Master’s degree in nursing is preferred/required.
• 5+ years of broad based clinical nursing and 2 years of utilization/case management experience are expected.
• BSN within 1 year if not already held may be required.

Responsibilities

• Coordinate care across an episode of illness and manage transitions from inpatient to rehabilitation or home.
• Educate and empower patients and families to self-manage and navigate the healthcare system.
• Promote safety, quality improvement, and adherence to post-discharge plans.
• Document care, communicate with the care team, and support discharge planning.
