Job summary

Your Health is seeking a Transitional Care Professional to coordinate patient transitions from hospitals and facilities to home. You will perform home and facility visits, ensuring seamless services and communication across care teams to reduce readmissions.

A successful candidate will have RN licensure, strong communication, and the ability to manage multiple responsibilities while traveling between offices and facilities in the Charleston area.

Qualifications

• RN license or equivalent preferred.
• Experience in public health or home health is a plus.
• Strong communication and leadership abilities required.

Responsibilities

• Visit referrals and established patients in assigned facilities.
• Build trust with patients to support informed health decisions.
• Coordinate care and arrange home health/outpatient rehab as needed.
• Schedule all essential visits with providers, pharmacists, social workers, etc.
• Communicate effectively among healthcare professionals involved in care.
• Ensure adherence to the care plan by patients and families.
• Provide compassionate customer service and address concerns promptly.
• Perform general administrative tasks and use the company’s software systems.
