Job summary

Your Health Organization in the Charleston area is seeking a Transitional Care Professional to coordinate patient transitions from hospitals and facilities to home, ensuring smooth, coordinated care. The role involves visiting patients in homes and facilities, daily travel, and working full-time 8-hour shifts Monday–Friday; strong communication and caregiving skills are essential.

You will collaborate with care teams to arrange home health, rehab, and follow-up services, and document visits

Qualifications

• RN license preferred and in good standing
• Minimum of three years clinical experience preferred
• Strong communication, leadership and organizational skills
• Ability to travel between offices and facilities; reliable transportation required
• Ability to read and communicate effectively, with good written skills

Responsibilities

• Visit referrals and patients within assigned facilities
• Build trust with patients to support informed health decisions
• Coordinate home health, rehab and follow-up services
• Schedule essential visits with primary care, specialists, pharmacists, and other staff
• Facilitate communication among healthcare professionals involved in care
• Ensure patient and family adherence to the plan of care
• Interact with patients via phone, email, portal, or in person to gather information
• Provide compassionate customer service and address concerns promptly
• Perform general administrative tasks and support staff as needed
• Utilize the company software to enhance patient care
• Be available during normal working hours and travel between facilities
