Job summary

Bluegrass Care Navigators seeks a Transitions Care Coach (RN) to support patients in Jackson, Laurel, and Whitley counties. This PRN role focuses on in-home visits, education, and care coordination to prevent hospital readmissions and improve chronic-condition management.

You will collaborate with patients and caregivers, identify goals, and navigate resources in community and home settings while maintaining HIPAA compliance and high-quality service.

Qualifications

• RN license in good standing with the applicable State Board of Nursing.
• 1–2 years of experience in home health, healthcare coordination, or community-based care preferred.
• Strong communication, interpersonal, and organizational skills for engaging with patients and caregivers.
• Ability to drive and conduct home visits in diverse settings, including private residences and facilities.

Responsibilities

• Provide individualized education and support to eligible patients across home and community care settings.
• Build trusting relationships to promote self-management, goal setting, and barriers removal.
• Coordinate care and resources to improve health outcomes and reduce hospitalization risk.
• Communicate clearly with patients, families, and healthcare professionals; maintain HIPAA-compliant confidentiality.
