Job summary

Your Health Organization is seeking a Transitional Care Professional to coordinate patient transitions from hospitals and facilities to home across the Charleston area. You will perform visits in homes and facilities, travel daily, and work full-time from 8am-5pm, Monday to Friday, supporting care teams to ensure smooth transitions.

Ideal candidates have RN licensure and at least 3 years of clinical experience, with strong communication and coordination skills to engage with patients, families,

Qualifications

• RN or Licensed Nurse preferred; certifications/licenses in good standing.
• Minimum of three (3) years clinical experience preferred.
• Ability to read and communicate effectively; strong written and verbal skills.
• Ability to supervise and direct professional and administrative personnel; organize and multitask.

Responsibilities

• Visit referrals and established patients within assigned facilities.
• Establish trust with patients to aid decisions during facility stay.
• Coordinate care and feedback with care team for patient plans.
• Arrange services for transition home (home health, rehab, follow-up).
• Schedule visits with primary care, virtual providers, specialists, pharmacists, social workers, etc.
• Facilitate communication among healthcare professionals involved in care.
• Provide compassionate, patient-centered service and address concerns promptly.
• Perform general administrative tasks and use company software.
• Must be available during normal work hours; travel between offices and facilities.
