Job summary

Your Health Organization is seeking a Transitional Care Professional to coordinate patient transitions from hospitals and facilities to home across the Charleston area. The role requires travel daily and involves home and facility visits within N.Charleston and Mount Pleasant service areas.

The position is full-time, with 8-hour shifts, Monday–Friday, 8am–5pm. Ideal candidates are RNs with strong care coordination, communication, and organizational skills.

Qualifications

• Registered Nurse with current licensure in good standing.
• Experience in care coordination or home health settings preferred.
• Strong communication and interpersonal skills required.

Responsibilities

• Visit referrals and established patients in assigned facilities.
• Coordinate transitions from hospital or rehab to home and arrange follow-up services.
• Ensure timely scheduling of visits with primary care, specialists, and support staff.
• Communicate effectively with care team and patients/families.
• Provide compassionate, patient-centered service and address concerns promptly.
