Qualifications

• RN licensure in Virginia or eligibility.
• Minimum 3 years of nursing in an acute care setting.
• BSN from an accredited School of Nursing is required.
• MSN or healthcare-related master's degree preferred.

Responsibilities

• Collaborate with physicians, social workers and discharge planners to coordinate care.
• Guide patient transitions through hospitalization to post-discharge care.
• Coordinate across the multidisciplinary team to meet patient needs and ensure appropriate reimbursement.
• Engage social workers to develop complex transition/discharge plans.
• Act as liaison with community agencies for referrals and information sharing.
• Comply with QIO and CMS guidelines and participate in patient safety initiatives.
