Job summary

Virginia Mason Franciscan Health is seeking a RN Care Coordinator to act as a central figure in patient care, guiding patients through the healthcare journey for optimal outcomes and exceptional experience.

You will assess, plan, and facilitate comprehensive care across the continuum, advocating for patients while collaborating with physicians, nursing, and post-acute providers to ensure timely, high-quality transitions.

Qualifications

• Graduate of an accredited school of nursing and minimum two (2) years of acute hospital clinical experience.
• Master’s in Case Management or Nursing with 1 year of experience.
• Registered Nurse licensure in WA.
• Basic Life Support – CPR within 90 days.

Responsibilities

• Complete and document discharge planning assessments and update plans as needed.
• Lead development of multidisciplinary discharge plans with patients, families, and post-acute providers.
• Oversee and evaluate discharge plan implementation.
• Collaborate with the care team to ensure appropriate inpatient resource use per guidelines.
• Coordinate with post-acute providers to ensure timely transitions.
• Identify readmission risks and implement mitigation strategies for at least 30 days.
