Job summary

Option Care Health seeks a Clinical Transition Specialist (RN or Pharmacist) to educate patients and care teams, coordinate transitions to home health or ATS, and grow referral sources. You will collaborate with intake, discharge planners, and account managers to ensure timely and effective treatment plans.

The role requires a licensed RN or Pharmacist with healthcare experience, strong communication, and the ability to travel to care facilities and hospitals.

Qualifications

• Experience building relationships with stakeholders at all levels in healthcare.
• Experience delivering quality customer service and meeting service standards.
• Ability to plan, organize, and present information clearly.

Responsibilities

• Proactively maintains and grows referrals to increase sales and patient starts.
• Interacts with area service providers to sell services and assess patient suitability for alternative-site care.
• Educates patients, families and facility staff about Option Care services and products.
