Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and families about home health services at an alternate site and ensure proper patient placement. The role builds referral relationships and collaborates with internal and external partners to grow sales and meet goals.

The position requires an RN or Pharmacist license with at least 2 years in healthcare, strong relationship-building, and solid PC skills. Travel to care facilities and hospitals is expected.

Qualifications

• Licensed RN or Pharmacist in the state of practice.
• Minimum 2 years in healthcare industry.
• Experience establishing relationships at all levels of an organization.
• Proficient PC skills: Word, Excel, PowerPoint.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell services and assess patient candidacy for home care.
• Educates patients, families and facility staff about services and products.
• Provides hands-on, in-person education to patients and caregivers for timely discharge.
