Option Care Health, the nation’s largest home infusion provider, is hiring a Clinical Transition Specialist (RN) in San Antonio, TX. You will educate patients, families and facility staff on services, coordinate pre-admission information, and build relationships with referral sources to drive patient starts and referrals across sites.

The role emphasizes sales-driven care transitions, collaboration with internal intake and external case managers, and ensuring seamless patient discharge to home

Qualifications

• Licensed RN in the state of practice with at least 2 years in healthcare.
• Experience building relationships with stakeholders and vendors.
• Able to travel to care facilities, hospitals and client sites.

Responsibilities

• Grow referrals and educate patients and facilities about Option Care services.
• Coordinate care transitions from hospital to home or alternate site.
• Collaborate with Account Manager to execute area business and growth plans.
