Job summary

Option Care Health in Texas seeks a Clinical Transition Specialist, an experienced sales professional who educates patients, families and facility staff about home infusion services. You will build relationships with referral sources, collaborate with intake and discharge teams, and work with Account Managers to grow referrals and meet sales goals.

The role requires an RN or licensed pharmacist with at least two years in healthcare, plus strong communication and travel readiness to support care

Qualifications

• Requires RN or Pharmacist license in the practicing state.
• Minimum 2 years of healthcare experience.
• Strong communication and relationship-building skills.
• Ability to plan and present effectively.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell services and assess patient suitability for home care.
• Educates patients, families and facility staff about services and products offered by Option Care.
• Provides in-person education to patients and caregivers to support timely discharge and independence.
• Communicates with agency staff, medical teams, patients and families throughout the discharge process.
