Option Care Health is seeking a Clinical Transition Specialist to drive sales of home infusion services by educating patients, families, and discharge staff about offerings at alternative care sites. You will build and nurture referrals, coordinate with hospitals and case managers, and support timely discharges to home care.

The role emphasizes strong relationship skills, healthcare knowledge and the ability to work with Account Managers to achieve sales goals across multiple facilities.

Qualifications

• Proven ability to build and maintain relationships with referrals sources and internal teams.
• Experience selling services to discharge planners, clinicians, and patients.
• Understanding of ACHC/URAC standards and healthcare compliance.

Responsibilities

• Grow referral sources and patient starts through proactive outreach.
• Educate patients, families, and facility staff about services and products.
• Assess patient suitability for home/in alternative site care and coordinate transitions.
• Collaborate with account managers and regional leadership to execute growth plans.
• Ensure timely discharge and patient education to support therapy independence.
