Job summary

GI Alliance in South Portland, ME is looking for an experienced Patient Navigator RN to guide patients and families from diagnosis through care, explaining options and offering support at every step. You will be the liaison between patients, providers, and staff to ensure seamless coordination and documentation.

Responsibilities include keeping medical records updated, coordinating PRE- and post-treatment steps with the team, and staying current through professional education while maintaining

Qualifications

• Graduate of an accredited nursing program.
• Current RN license and any state certifications required.
• BLS certification.
• Minimum 2 years of clinical experience.

Responsibilities

• Guide patients and families through their healthcare experience, explaining diagnosis and treatment options.
• Follow patients from diagnosis through completion of their care path to ensure all options are explored.
• Review and update patient history and medical records promptly.
• Collaborate with surgical scheduling on prep, clearances, and complex procedure issues.
• Raise workflow problems or concerns with clinical services leadership.
• Stay current through workshops, publications, and professional societies.
• Support HIPAA compliance and annual compliance training.
• Take on other related duties as assigned.
